As far as customizing your keyboard goes, you can actually choose to change your key size, the key alignment, and more. You can adjust things to your personal tastes, and then overlay an emoji on whichever key(s) you want.
Overall, you're not getting anything but the emoji library access in addition to being able to customize your keyboard.
